Participates in rotating Intake on-call schedule.
The Scheduler Intake Coordinator works in a multi-faceted role, encompassing responsibilities beginning at the referral time point and continuing throughout a patient's episode of care.
LVN Coordinator facilitates the intake of patient referrals, insurance verification, and authorization, and scheduling of patient visits.
Key Performance Outcomes - Ensures timely, accurate data entry of incoming referrals.
- Maintain full-time field clinician productivity of at least 90%.
- Effective in managing multiple concurrent tasks.
Key Responsibilities - Receives and documents new referrals, from a variety of sources, in a timely manner and communicates incoming referral information to management.
- Verifies insurance benefits, obtains prior authorization for services, and appropriately documents information into EMR.
- Ensures all documents are in order and submitted timely according to Medicare, insurance, and best practice standards.
- Schedules patient visits in an appropriate, efficient, and business-conscious manner.
-- Monitors full-time staff productivity, ensuring compliance scheduling prioritization hierarchy.
- Provides effective, professional communication to patients, Agency team members, and referral sources.
- Participates in Intake on-call rotation.
Qualifications - LVN License - Reliable transportation.
Valid auto liability insurance.
- One year of experience in a Hospice Intake, Authorization, and/or Scheduling role is required.
- Excellent phone, computer, and interpersonal skills required.
